The Department cannot provide rosters with licensee addresses. The Civil Administrative Code was amended in 2021 to include that rosters must comply with FOIA. FOIA does not allow the release of addresses. The Department recommends utilizing the Bulk License Look-Up that is on the website, where you can type in up to 1,000 license numbers and the system will retrieve all of them. Bulk License Look Up has been approved for use as a primary source for verification by the Joint Commission of Accreditation of Healthcare Organizations and the National Committee for Quality Assurance.
